I think this is a known issue - look through Red Hat bugzilla perhaps? I
have a note about this dated Aug 2013, but don't have a reference
for you.
One solution is to try a script like this:
# cat /etc/sysconfig/modules/mlx4_en.modules
#!/bin/sh
/sbin/modprobe mlx4_en
We have found the Red Hat mellanox driver to be problematic, and
instead we do a kmod-style driver using driver code
from the mellanox web site.
