UW IMAP written by the author of the IMAP RFC stored multiple messages in the same file (one file per mailbox). It also offered the option of an indexed (binary) mailbox format for better concurrent access (less whole file locking). The way messages are stored on the server is an implementation detail. Having said that Exchange and Office 365 are awful in this regard.
